Leveson's book covers the history of safety engineering; explores risk, ethics, legal frameworks, and policy implications; and explains why accidents happen and how to mitigate risks in modern, software-intensive systems. It includes accounts of well-known accidents like the Challenger and Columbia space shuttle disasters, Deepwater Horizon oil spill, and Chernobyl and Fukushima nuclear accidents, examining their causes and how to prevent similar incidents in the future.
